Transitions Notice Information

Please submit information ONLY AFTER confirming service arrangements with the funeral home.

If you would like Beth Yeshurun to send a Transitions notice regarding the death of a member of your family, (or you are acting on behalf of the family), please complete the webform below. Once we have received the information, we will create a draft of the Transitions notice and send it to you for your approval. Following your approval, (after any requested corrections), the notice can only be sent once to everyone on our Transitions List.  If there is an error found after you have given us approval, we will be happy to update our website with the corrections.

Surviving Family Information

Surviving child and their spouse if applicable. If none, please enter "none" When appropriate, please indicate "step-son," "step-daughter," "son-in-law," or "daughter-in-law"
List of living parents and their spouse. If none, please enter "none"
Surviving sibling and their spouse. If none, please enter "none"

Funeral and Shiva Information

If you indicate that the funeral IS private, we will not release the date, time or location.

Cemetery in which the funeral will take place. Please note if the funeral will be held elsewhere (i.e. if it will be held at Congregation Beth Yeshurun).
Please list date(s), time(s), and place(s) including address(es), of any shivah minyanim already scheduled. Although this information does not appear in the Transitions notice, we do provide a link to the information within the notice. If you do not have this information yet, it may be added later. Example: Tuesday, Feb. 26, 2019, at 6:30pm at the home of Brenda & Morrie Rosenfield (1011 Main St., Houston, TX 77096) Please note: private addresses are not published on the website unless requested by the family, but will be released upon calling the synagogue.

Family Contact Information

Contact name and a valid email and phone number of the person who is submitting this information - or doing so with the permission of the family. A valid email is required as a draft of the Transitions will be sent for approval prior to transmission to Congregation Beth Yeshurun's "Transitions" list. Once the draft is approved, following any requested corrections, the notice can only be sent once to everyone on our Transitions List. If there is an error found after you give us approval, we will be happy to update our website with the corrections.
